Comparative effectiveness research in cancer screening programmes.

چکیده
In recent decades, cancer screening programmes (screening that is publicly organised and includes invitation procedures for eligible people of the average risk population in the screening area) have been established in many countries. While cancer screening in the context of clinical trials is innovative and investigative, cancer screening programmes themselves are largely static and not designed to generate new, evidence based knowledge. However, screening programmes themselves affect health and healthcare, which may in turn substantially affect the effectiveness of the programmes. Many screening programmes today can be regarded as supertankers; once under way, they are difficult to halt or alter in direction or content. In mammography, increasing concern about the benefits and harms of screening programmes has led to the announcement of an independent review of mammography screening in the United Kingdom. Here, we outline new approaches that aim to overcome this obstacle, using the principles of comparative effectiveness research. Screening programmes changemedicine

Colorectal cancer screening: efficiency and effectiveness.
The cost-effectiveness of a series of mutually exclusive colorectal cancer screening programmes with varying screening interval and target group are analysed. Costs and effects for 60 possible screening programmes are simulated on the basis of data collected from a randomized trial initiated in 1985 in Funen County, Denmark. The screening test applied is the unhydrated Hemoccult-II. The analysi...

Background Over the past few decades diabetes has emerged as an important non-communicable disease in SubSaharan Africa (SSA). Sight loss from Diabetic Retinopathy (DR) can be prevented with screening and early treatment. The objective of this paper is to outline the required actions and considerations in the planning and development of DR screening services. Methods A multiple-case study app...
متن کاملAlternative technologies in cervical cancer screening: a randomised evaluation trial
BACKGROUND Cervical cancer screening programmes have markedly reduced the incidence and mortality rates of the disease. A substantial amount of deaths from the disease could be prevented further by organised screening programmes or improving currently running programmes. METHODS/DESIGN We present here a randomised evaluation trial design integrated to the Finnish cervical cancer screening pro...
متن کاملPublic health genomics and personalized prevention: lessons from the COGS project
Using the principles of public health genomics, we examined the opportunities and challenges of implementing personalized prevention programmes for cancer at the population level. Our model-based estimates indicate that polygenic risk stratification can potentially improve the effectiveness and cost-effectiveness of screening programmes. However, compared with 'one-size-fits-all' screening prog...
متن کاملIs it cost effective to introduce screening programmes for colorectal cancer? Illustrating the principles of optimal resource allocation.
This paper seeks to shed light on the relative cost effectiveness of colorectal cancer by comparing the cost effectiveness of this programme with the economics of another screening programme which is widely implemented: cervical cancer screening. The paper illustrates the principles of optimal resource allocation, and discusses the limitations and strengths of the analysis presented. The paper ...
